AMM Registration Form (Rich Text File) which may be opened in any word processor once downloaded. Follow the directions at the top of the form and fill it out. Once the form is completed, follow the directions at the bottom of the form to send it to a secure email account at the USAWOA Home Office.
For those of you who do not desire to use the electronic version -
Print/Download the paper version of
AMM Registration Form
for mailing or faxing to the Home Office;
Recommend you use your AKO or other military email for added security of your personal data. Use of commercial email to send the form as an attachment provides a relative amount of security for your personal data unlike text in the body of an email, or a text file attachment.
